Both engines failed within a short interval 18 minutes after takeoff from Bario Airport, Malaysia, on a 35 minutes sector. The pilot attempted to land on a hillside jungle track used by timber transporters. The wings struck several trees causing the aircraft to impact nose down onto the hill slope.

The engines failed due to lack of fuel, resulting from the crew failure to ensure enough fuel for the flight. Contributory cause was the absence of a proper regulatory body to monitor the operator's activities through regular inspections and continuous surveillance.

Causal Factors
The main cause of the accident was the double engine failure in flight due to lack of fuel, resultant from the crew failure to compute adequately the flight fuel requirement and to ensure correct amount of fuel in the aircraft tanks before each flight.

The contributory cause of the accident was the failure of the aircrew and the Operator to observe the operational requirement especially the fuel computation due to lack of supervision by the Authority and proper independent entity to inspect and certify the operators activities during the initial issuance of Permit and the continuing surveillance on the operators.
