PROBABLE CAUSE:THE INFLIGHT SEPARATION OF THE TAIL ROTOR DUE TO IMPROPER MAINTENANCE PROCEDURES AND THE INSTALLATION OF INCORRECT ATTACHMENT HARDWARE ON THE TAIL ROTOR BLADE PITCH CHANGE LINKAGES.
