Narrative:
Observation mission whose task was to carry out a reconnaissance and aid flight to the victims of the huaycos in the rural area of ​​Ancash. The mission was made up of four civilians and three soldiers.

This mission departs from the air base of the Air Group No. 8 in Callao, with a stopover in Chimbote towards the city of Cabana, in the department of Ancash, taking with it the civil engineer Juan Jaime Pisconte Pérez, the police inspector Juan Luis Arbulú Pineda, two social workers from the National Assistance Board (JAN), Miss. Dora Tejada Triveñoz and Miss. Nelly Baca Gutierrez and three soldiers, Major Isaac Javier Tryon Cuba (Pilot of the BELL UH-1D helicopter marked with the number No. 628), Lieutenant Walter Balarezo Balarezo (copilot) and 2nd Sub-Officer. Juan Teobaldo Alva Murrugarra (ship's mechanic).

The helicopter descends in the town of Tauca, province of Pallasca, Department of Ancash. When the doors of the ship opened, the population came up to say hello. When you find out what the town is called, they tell you that they have landed in Tauca and not in Cabana, a town that is a few minutes away by flight, which is why they close doors and carry out maneuvers to ascend, suffering problems with a decrease. considerable power (Revolutions per minute – RPM) and precipitation of the helicopter to the ground. In this way, when making the emergency landing, a ski of the ship collides with the ground and the helicopter bursts into flames, instantly killing the ship's pilot (Major Javier Isaac Tryon Cuba) and its mechanic (Sub- Officer 2nd Juan Teobaldo Alva Murrugarra), in addition to the two social workers, with the survivors being Lieutenant Walter Balarezo Balarezo and two civilians, the civil engineer Juan Jaime Pisconte Pérez and the police inspector Juan Luis Arbulú Pineda.
