Narrative:Crashed into the sea after an unsuccessful night catapult launch off the USS Forrestal in the Mediterranean Sea.
Night operational reconnaissance flight/ASW training mission off of USS Forrestal.
Four minutes after catapult launch on climb out at 4,000’ pilot observed No. 2 engine fire light. Throttle placed in idle with fire switch. Fire light persisted and EGT climbed to above 950 degrees. During turn back to CVA-59 pilot observed Hyd Caution, Pitch Aug and Electric Flight lights illuminate. Pilot experienced loss of control and told RAN to eject. Stick and rudder inputs had no effect.
RAN ejected at 4,000’, 150 degree bank right wing down, 45 degree nose down and 300 KIAS. Pilot ejected 3,000’, inverted, 60 degree nose down and 350 KIAS. Both rescued by SH-3Ds 4 NM aft of ship with minor injuries. Aircraft wreck lost at sea in 2,500 fathoms (15,000’).
