Destroyed in combat 24 August 1970: shot down while on a recon mission in the Bien Hoa area, Đồng Nai Province, North Vietnam. While in Operations Area this helicopter was at Level Flight at 0050 feet and 020 knots. Hit by Small Arms/Automatic Weapons; Gun launched non-explosive ballistic projectiles less than 20 mm in size. (7.62mm). Count of hits was not possible because the helicopter burned or exploded.

Although the OH-58A Kiowa was hit in the cockpit area, crashed, and burned, and the pilot was killed, two others aboard survived with injuries. Contemporary press reports named the sole KIA fatality as WO1 Patrick G. Fitzsimmons [Pilot]
